Assumption College Returning Student-Athlete Forms

Returning Student-Athlete Health Status Report Form (fill out online)
All returning student athletes are required to fill out the health status report form. Any student entering their initial year of eligibility, transferring from another institution, or who has taken more than one full semester off from participating must fill out the first year report form below. The NCAA does not require documentation of an annual physical to continue playing intercollegiate athletics. The Assumption Sports Medicine Department strongly encourages student athletes to have annual physicals performed by their PCP.

The Health Status Report Form, along with the additional forms listed below, need to be completed prior to participation in intercollegiate athletics.
