We are pleased that you've become part of the Assumption College community and, in particular, as a new student-athlete. As you begin preparation for your first year there are several items (Sports Medicine, Initial Eligibility & Athletic Communications) that need to be addressed as soon as possible.
SPORTS MEDICINE INFORMATION
STUDENT-ATHLETE HEALTH HISTORY FORM
All student-athletes are required to fill out the student-athlete history form before he/she begins his/her first season of competition at the College. Just click the URL above, download the PDF 10-page questionnaire and fill out the form completely. Then mail it to: Sports Medicine, Athletics, Assumption College, 500 Salisbury Street, Worcester, MA 01609.

ADD-ADHD MEDICAL EXCEPTION FORM
The NCAA mandates that documentation for any student-athlete being medicinally treated for ADD or ADHD be filed confidentially in the Sports Medicine Dept. This downloadable PDF must be signed by the student-athlete's treating physician and must be on file in the Sports Medicine department in the event the individual tests positive during an NCAA drug test. If this documentation is not on file prior to testing positive they may not be granted a Medical Exemption. The form needs to be completed every year by all student-athletes being treated for ADD or ADHD.
HIPAA DISCLOSURE CONSENT FORM (PDF)
The student-athlete must authorize Assumption College and its physicians, athletic trainers and health care personnel to disclose my protected health information and related information regarding any injury or illness during my training for and participation in intercollegiate athletics. This downloadable PDF must be signed and returned to the Sports Medicine office.
MEDICAL REPORT FORM
This form must be completed and sent to Student Health Services in order for you to obtain your room key and attend classes. Please note that the dated physical that is required for admission to the College is not as stringent as what is required by the NCAA. As a student-athlete, you will need a physical dated after March 1 in order to be eligible to participate in your respective intercollegiate program.

INITIAL ELIGIBILITY INFORMATION
If you want to participate in intercollegiate athletics at Assumption College, you need to be certified by the NCAA Eligibility Center. You need to qualify academically and you need to be cleared as an amateur student-athlete. You are responsible for achieving and protecting your eligibility status.
