Assumption College student-athletes participate in 23 intercollegiate sports, the most of any institution in the 16-member Northeast-10 Conference.

Required Assumption College Student-Athlete Forms
- Athletic Communications Questionnaire (FOR CURRENT
ENROLLED STUDENT-ATHLETES ONLY, NOT FOR RECRUITS!)
(This is to be filled out EACH YEAR BY ALL
CURRENT STUDENT-ATHLETES. Please fill this out as
completely and accurately as possible. Please note that you must be
click SUBMIT, then review the form and hit SUBMIT a second
time.)
- Returning Student-Athlete Health Status Report Form
(Returning Student-Athletes ONLY)
All returning student athletes are required to fill out
the health status report form. Any student entering their
initial year of eligibility, transferring from another institution,
or who has taken more than one full semester off from participating
must fill out the first year report form below. The NCAA
does not require documentation of an annual physical to continue
playing intercollegiate athletics. The
Assumption Sports Medicine Department strongly encourages student
athletes to have annual physicals performed by their PCP.
- First-Year/Transfer Student-Athlete Health History Form
(PDF) (First-Year/Transfers ONLY)
All first year student-athletes are required to fill out
the student-athlete health report form before he/she may begin
their initial season of competition at the college. Student
athletes who are transferring to Assumption, or took more than one
full semester off must also complete this form. Just click
the URL above, and download the PDF. The first page has
information and a checklist regarding all of the first year
requirements. Once completed in full mail packet to: Sports
Medicine Department, Assumption College Athletics, 500 Salisbury
Street, Worcester, MA 01609. Or fax it to (508) 767-7140.
This form will satisfy the medical requirements of the
College’s Student Health Services
Department.

The emphasis for the student-athlete experience in
Division II is a comprehensive program of learning and development
in a personal setting. The approach in Division II provides growth
opportunities through academic achievement, learning in high-level
athletics competition, and development of societal attitudes in
service to community. The balance and integration of these
different areas of learning opportunity provide Division II
student-athletes with a path to graduation, while cultivating a
variety of skills and knowledge for life ahead.

Compliance Information
All student-athletes at NCAA Division I and II
institutions must have clearinghouse certification before playing
in college. It is recommended that you contact your high school
guidance counselor for more information on the process.
FIRST-YEAR STUDENT-ATHLETES, PLEASE NOTE: if you want to participate in intercollegiate athletics at Assumption College, you need to be certified by the NCAA Eligibility Center. You need to qualify academically and you need to be cleared as an amateur student-athlete. You are responsible for achieving and protecting your eligibility status.
